PRN Home Health Licensed Practical Nurse (LPN)

What You’ll Do

As a PRN Home Health LPN, you’ll provide skilled nursing services under the direction of an RN or physician by:

• Reviewing and following the established Plan of Care prior to each visit

• Delivering care according to physician orders, RN delegation, and clinical standards

• Incorporating patient-specific goals into each nursing visit

• Monitoring patient conditions and reporting changes to the RN Case Manager or physician

• Assisting patients with self-care techniques and disease management education

• Supporting patient safety initiatives and identifying environmental risks

• Documenting skilled nursing visits, medications, and patient responses accurately

• Collaborating with the RN Case Manager during business hours for guidance and support

• Implementing strategies to help prevent avoidable hospitalizations and ER visits

• Communicating professionally and compassionately with patients, families, and care teams

What You’ll Need

• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing

• Current, unrestricted State LPN license

• Minimum of 1 year of clinical nursing experience

• Home Health or Hospice experience preferred

• Ability and willingness to travel within the coverage area (approximately 50%)

• Valid driver’s license, reliable transportation, and current auto insurance

• Ability to sit, stand, bend, lift, and move intermittently, including lifting up to 50–100 lbs
